Practice Policies

We ask that all new families read our policies before the first visit. It contains descriptions of our appointment, insurance, financial, and referral, form, and prescription re-fill policies.

Late Policy

Compassion Pediatrics strongly enforces our 10 Minute Rule. If you arrive 10 minutes or more late for your Well Child Appointment you will be rescheduled. The reason for this rule is to keep as close to the schedule as possible. Please be courteous to those who are scheduled after your visit by arriving on time.

Cancellations need to be made as soon as possible. If the cancellations is not at least 24 hours prior to the appointment it is considered a no show.

Vaccine Policy

Our current vaccine policy allows parents to have the flexibility to alter the schedule they would prefer, but yet requires that all the children be vaccinated by 5 years of age.

Forms

On this page you can find downloads for all the important forms you’ll need for school, camp, and more.

1

New Patients

We have compiled a hard copy of Compassion Pediatrics’s policies. We ask that all new families read this and sign it at the first visit. It contains descriptions of our appointment, insurance, financial, referral forms and prescription re-fill policies. Click here to download this form.

2

Transferring Patients

Compassion Pediatrics requires that you have a full copy of your child’s medical records including any lab work done and all vaccines given. We will not register your child without the records. We suggest you contact your current pediatrician and have him send them to you and you then bring them to us. In that way, you will personally know the status of the chart. School & Sports Forms

Physical exams for school or camp forms are valid for one year. If your child has not had a physical within one year, please schedule one as soon as possible. Note that from April through September we get busy with camp and school physical appointments, so try to schedule an appointment as soon as possible, particularly if your child is behind on his/her well child visit.

Physical forms and shot records are given at the time off the well visit, except new patients may not receive an updated vaccination form at the time of visit - the pin number will be emailed within 48 hours. If additional forms are requested, they are $5/piece and requested by emailing info@compassionpeds.com.

Sport forms not completed at the time of physical are $10. Forms dropped off have a 72 hours turn around.
